Thank you for choosing our practice as your cardiology specialist.  We are committed to your treatment being successful.  In order to keep our fees under control, the office has established the following financial policies:

Usual and Customary Rates

Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area.  You are responsible for payment regardless pf any insurance company's arbitrary determination of usual and customary rates.

Payment

Full payment is due at time of service unless we participate in your insurance plan.  We accept cash or check.  We also offer an extended payment plan with prior credit approval.

Co-Payments

All health insurance plans in which we are participating in will be billed first, with the patient being responsible for any co-payment and deductible amounts. This co-pay is due at the time of your visit.  

  • Direct billing will be done for all Medicare patients. 

  • Medical assistance patients will be responsible for a co-payment amount due at the time of each visit.

Non-Participating Insurance Plans

Payment for office visits is expected at the time of service.  Surgical procedures will be billed directly to your insurance carrier.  We will provide them with all required information.  Your insurance policy is a contract between you and your carrier.  Please be aware that some services provided are not covered by insurance.  This varies according to your insurance contract.

Participating Insurance Plans

All co-pay amounts are due at the time of treatment for office services. Surgery co-pay amounts and deductibles will be billed  and due in "full" in 30 days from the statement date. In the event your insurance coverage changes to a plan we do not participate in, refer to the above paragraph.

Insurance Information

We ask that you bring your health insurance cards with you to the office. This will ensure correct billing  information. 

Managed Care Plans

If your coverage is provided through a managed care program, please be sure to acquirer a referral form from your primary care physician for the visit and bring it along to the office.  We are no longer provided with after-the-fact referrals from primary care physicians.  Without this form could cause a delay in your appointment.
